Audit logging coverage and diagnostics

Summary

• normalize hook telemetry through shared helpers so injections and outcomes log consistent coverage metadata for Edit, Write, and NotebookEdit paths
• harden the telemetry dashboard and shell tests around uncovered writes, malformed rows, opt-out behavior, and log rotation
• make eval-harness phase logs deterministic per task/condition/rep and include log paths in timeout and infrastructure diagnostics
