Summary
This PR updates the goal continuation prompt to address feedback from early adopters. In particular, it makes persistence explicit, grounds work in current evidence, encourages update_plan for multi-step progress visibility, and requires stronger completion audits before calling update_goal.
This also fixes the prompt renderer after removing elapsed-time reporting: continuation_prompt no longer supplies the removed time_used_seconds template variable, preventing goal creation or continuation from panicking on the strict unused-variable check.
