Fix Clone Loop AskUserQuestion threshold gating

Summary

• enforce Clone Loop AskUserQuestion threshold/status gating before auto-answering
• remove deterministic first-option fallback for unmapped predictions and MCP failures
• keep loop state active when question prediction is unsafe so Claude asks the user normally
