Resolves issue #22.
CombinedFieldandAdjointCombinedFieldOperators/Kernels/Potentials were implemented.Now the DIM singlelayer/doublelayer (adjointdoublelayer/hypersingular) corrections are computed in the same loop. I'm not sure if the compiler can guess that the loop is the same in both cases, so now the corrections are explicitely computed in the same loop just in case.
