…tation Renames escalations.mdx to alerts.mdx to match the product UI, documents all three alert types (status change, tool error, agent published), notification channels (email, Slack, Teams), and the escalate to manager tool. Moves tool approvals and failure policy content to the tools page where it belongs in the product. Adds redirect for old escalations URL.
