Updates for settings for pyOCD Trace

Fixes

• Part of [Feature Request]: Concept for Trace Port Configuration vscode-cmsis-debugger#581

Changes

• Remove port-width for pyOCD, not expected to be supported there soon (effectively required for sync trace port)
• Be more specific about configured trace output referring to raw trace output ("formatter level" if formatter enabled).
• Minor clarifications of modes for which settings are relevant.
