Changes: - Don't start up an application, instead the user starts it as part of their supervision tree - Don't rely on the application environment directly - Allow the user to implement a function to decide what to do with a request - Allow passing a domain to check Still needs cleaning up and possibily splitting into multiple PR's

I'm creating this PR because this is what I'm using on my Nerves Vultr and Nerves Side Screen projects, but I'm keeping this as a draft PR because I plan to re-write this code.
