Description
This PR adds verification form (UI only).
Verification form consists of 4 steps: About verification, Personal data, Documents, References. User can go to next step by clicking on continue button.
Form validation and error message will be added in next PR.
Form is accessible at /incorporation/verification route. A redirect to this page will be added in a separate PR.
Next steps of the form (with sign field and confirmation page) are not included in this PR.
