The lines that start 3: I can guess at but what are the +... lines on about?

It's an approximate measurement of the gap in "us" (microseconds), which can be useful for estimating baudrate, etc. It's only logged when the polling loop sees no change.

There's no documentation other than the usage information and whatever is in the README.

Yes, approximately - each gap will be a few microseconds out.
