Modify KG Award Form

1. It should stress on the top the details are for the candidate and not
   for yourself (person submitting the form).
2. "Postal address" - It says "Your Full Address" which is wrong. It
   should be "Full address of the candidate". I believe this is optional
   and instead a "City" field to be introduced which should be compulsory.
3. "Profession" - "I work study at..." is again confusing. It should be
   "Profession of the Candidate" and can be a list item drop-down.
4. References can be made optional I believe.
