[Bug]: Keeps tracking stale opencode sessions long after they become inactive

Steps to reproduce

Not guaranteed. Explaining what I did.

1. Query opencode with Opencode Go + Qwen 3.6 plus
2. steer with few more queries
3. Infinite session.

Expected behavior

Should be in sync with opencode and stop when the session stops the work.

Actual behavior

The session stays in pending state and stop button does nothing. Closing and reopening also doesn't do anything.

Workaround

After quiting the T3 code completely and steering with cmd + enter able to make a query on this session

Is it possible to have a button top open the current session in opencode TUI from t3 code?
