Form can not be saved

Describe the bug
Form can not be saved. If I refresh the tab the Header says New Form and all settings are gone.

@xrsc6sxk9w-sys could you please check if you're missing some DB columns? If this is not the case, please provide us with relevant logs from the browser console or the Nextcloud server log.

If this is not possible please add at least the steps needed to reproduce the error.

If non of this is given, we can't do anything...

I am encountering the same issue. When trying to save or update a form, the server returns a 500 Internal Server Error. The logs show a missing database column in the forms_v2_forms table.

Environment:

Nextcloud version: 31.0.11.2
Forms version: 5.2.3

Log output: The PATCH request to /ocs/v2.php/apps/forms/api/v3/forms/{id} fails with the following error:

Troubleshooting performed: I have already tried the following occ commands without success:

• maintenance:repair
• db:add-missing-indices
• db:add-missing-columns (Reported "Done", but did not fix the table)
• Disabling and re-enabling the app.

It seems the migration responsible for adding locked_by and locked_until to the forms_v2_forms table did not execute or failed silently during the update.
