Proposed documentation enhancement

it would save maintainer time if we had boilerplate guidance to point to on how to do this. Example:

  • make new local branch based on existing stale fork/branch, then add your commits on top, then push to your fork/branch

other (simpler) possibilities include, e.g.

  • add stale PR author to changelog entry and make them (co)author on that commit

could also explain how to do it after-the fact (e.g. rebase against stale PR)
