geant4 11.2.2 mutex lock failure bug

When upgrading to geant4 11.2.2 we got hit by

The workaround is to move to G4MTRunManager with a single thread. I wasted a few days trying to do that, before finally aborting the effort since NCrystal does not support it, even for a single thread.
