handle errors on add/start/stop/delete

In manager.go, during Add/Start/Stop/Delete, errors can happen:

• create/start/stop/delete runtime instance fails
• saving the new runtime instance status to instances.json fails (file read-only, etc)

We should provide a way to handle these errors gracefully
