copy the php file under my tycket plugins directory ?

when you write "Cut and paste the code into a new file" you are talking about for exemple am-change-notification-email.php which is into the directory "access-monitor

but what about the second file ?

I'm not sure what you mean by "the second file" - can you clarify?

Any of the files inside the plugin-extensions property are independent plugins; they should be installed in your main plugin directory at /wp-content/plugins/, and should not be nested inside any other plugin directory.

That said, Access Monitor is a dead plugin, as the service it interacts with is no longer operating, so those specific examples are probably things I should remove...
