[feature request] Ability to customize the prompt.

I've noticed that you've been updating the prompt, and while I love some of the changes, there are other bits which I don't like as much. (10. If the diff includes multiple unrelated changes, pick the single most important one)

As such, it would be really nice if there was a way to customize the commit message prompt a bit, either with a full replacement, or by being able to adjust specific rules.

I'm not really a fan of how I handled the case of a nil prompt_template string, but the substitutions work reasonably well, and at least for my use case this is letting me customize the prompt as I want to.
