Individual: link to Household and Services

User wants to link multiple Individuals to a Household or Service at a time

Acceptance Criteria

GIVEN I am a logged in user

AND I navigate to the Form library > Individual page

WHEN I check on the select box next to the names of the Individuals

THEN I see the option to assign a Household and Service from the dropdown list
