Add updated/updaters field to Surveys

Right now it's impossible to tell if a Survey record has been edited by a commander or if the existing entries are all from the Monster spreadsheet.

We need to add the updaters and updated_by columns just for tracking changes. The updater must still be the owner (commander) or a surveyor (when #12 is implemented) when a change comes in from a web request.
