Add input box for user to provide their name before connecting to video

Currently the user's name is hard-coded as "the-other-user". This is shown in the invitation accept/decline dialog.

Both users should be able to specify their names so the other user knows who they are, to provide a more human experience, and for authentication (i.e. Alice can choose to decline an invitation from Frank if she was expecting an invitation from Bob).

The name will be used in the following scenarios:

1. When Bob clicks "Connect to Video", his name will be shown to Alice before she accepts/declines the invitation.
2. After Alice accepts the invitation and video/audio connection is established, both users' names will be shown to each other as a label for the video.

Implement name input as a div or p element that becomes editable upon clicking the text and submits onblur (enter should trigger a blur event).
