Record client-side tool invocations for improved auditing

Problem

AI Bridge only records injected MCP tools' results. Client-invoked tool results aren't captured, so failed tool invocations only appear in client-side transcripts/sessions, not in AI Bridge interception logs.

Solution

Intercept tool result messages and record them.
