ai-devkit agent list command showing wrong status of Claude Code agent

1. run multiple Claude Code in different tmux panel
2. run ai-devkit agent list

it is showing duplicated agent of same process, ex:

feature-agent-send                   🟢 run  Claude process running
feature-agent-send (pid-9181)        🟢 run  Claude process running
feature-agent-send (pid-9182)        🟢 run  Claude process running
feature-agent-send (pid-9182)        🟢 run  Claude process running
