We should have a form to enter criteria instead of hard-coding values

Instead of hard coding a username or a role or a maximum number of results into the PHP code, we should provide an interface to enter that information and then after submitting the query we can display the results.

This would be nice if we had a flag to indicate whether we want to see the raw JSON or if we want to format the results. Then, I suppose we would need to address how we want to format the results... table, list, full info, summary, etc.
