Create different issue templates

E.g. for workshop, bug report, etc.

For workshop - offer people pre- and post-workshop surveys to copy from (also add them to the README).

Can we have a 'feedback' template, which adds a 'feedback' tag to the issue - then we can filter these out when looking for tasks for the next release.
