Usage limit retry countdown displays incorrect time and lacks model identification

Description

The usage limit error message shows a retry countdown that doesn't align with the actual reset time, and the error message doesn't specify which model/provider has hit the limit.

Usage limit reached for 5 hour. Your limit will reset at 2026-04-11 20:30:24 [retrying in 2m 22s #8]

The retry countdown should either:
Show the actual time remaining until reset (e.g., "retrying in 11h 23m")
OR not show a countdown if the reset is >1 hour away
OR use exponential backoff instead of fixed intervals
The error message should include the model name and provider:

Usage limit reached for Claude 3.5 Sonnet (Anthropic) - 5 hour window.
Your limit will reset at 2026-04-11 20:30:24

Also if you switch models you cannot force continue on the new model
