Add Mentee Synopsis for Mentorship Program

Add the following to the Mentorship Guidebook https://github.com/OperationCode/operationcode_docs/blob/master/mentorship/GUIDEBOOK.md

1. Add section for General and Custom Career Guidance, UI/UX and Interactive Design so that mentees also know there are other options available other than Software Development in the intro.
2. Add the following as a sample template that the Mentors can request Mentees to complete prior to the first mentorship meeting. This will help provide the mentor with background information so the mentorship session is as productive and tailored as possible:
