Feature Request: Notification after transition/state change

As a developer, I want to be able to add observers either to a reached target state or a completed transition, so that I can trigger any follow-up actions, e.g. "mail notification after account upgrade".
Any thrown exceptions during the execution of follow-up observers shall be ignored.
