Terminating Pending Orchestrations Scheduled by Entities (Azure Storage)

Currently, it is not possible to terminate pending orchestrations scheduled by entities since no instance record is created for these orchestrations until they start running in the Azure Storage backend. A fix would be to create the instance record for entity-started orchestrations as well.
